Intellectual Disability/Autism Spectrum Disorder (ASD) service- To gain services you may request to speak with an intake manager at 610-236-0530 and fill out
Person/Family Directed Support Waiver and Community Linking Waiver and Consolidated Waiver. Once determined eligible the support coordinator will assist you in determining how your service will be funder. Mental Health County Funded Treatment and Support Services- they offer many different services and to gain these services you may contact them at 610-236-0530 and ask to speak to the Mental Health Intake Department. Health Choice Program care program is for people who are already receiving Medical Assistance (Medicaid) and all eligible participants are required to participate in the HealthChoices program. Early Intervention Services-children from birth to three are eligible if they have a delay in development in one or more areas of development, a physical disability, or has a condition that has a high probability of developmental delay. How to request services is by contacting service access and management and requesting an early intake for children under the age of three at 610-236-0530. Children and Adolescent Service System Program (CASSP)-this service helps children, adolescents, and their families gain access to needed services. Emergency Behavioral Health-they provide procedures to ensure an effective response to the Berks Community after a disaster or emergency.
